Initial Comments:
An off-site, state licensure follow-up survey conducted on January 4, 2024 found that Case Community Anchored Service Exchange, LLC had corrected the deficiency cited under 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H, Home Care Agencies and Home Care Registries. The deficiency was cited as a result of a state re-licensure survey completed on November 14, 2023.
